Resource Forms
These forms are used in our personal counseling ministry. In order to serve you promptly and efficiently, these forms give us substantial background information, which saves time during the actual counseling process. Prior to our first meeting, Grace Harbor Counseling Ministries requires these forms to be filled out and sent back to the GHCM office. You may send the completed forms to GHCM electronically or via the Postal Service.
If you are using these forms in conjunction with GHCM counseling, please do not alter them. However, if you wish to use these forms for your own ministry, you may do so. In such cases, please remove all references to GHCM.
